IHVN is a leading provider of treatment, care and support for  people living with HIV/AIDS with headquarters in Abuja and Regional  offices in Benin, Jos, Kano and Federal Capital Territory. We require a competent and result-oriented candidate to fill the  position below. 
Education/Experience:
Basic General Nursing Certificate, Basic Midwifery and/or  any other post graduate Certificate in Nursing, BSC Nursing or any of  the Social Sciences with excellent computer skills. An MPH will be an  added advantage At least 10yrs working experience and have participated  in both clinical and community activities. Familiarity with the Nigerian  health system and social networks. 
Key Areas Of  Responsibility
 -  Develop and supervise provision of services that assists clients to obtain and coordinate community resources for health education, medical care, treatment, spiritual care, psychosocial and personal support systems.
-  Assist facilities to develop systemic identification of client need and, through a system of referrals, link clients to the resources or services required.
-  Ensures that facilities provide all HIV+ patient enrolled in primary care with associated services such as nutritional assessments, mental health interventions, treatment adherence support, prevention education, disclosure counseling and partner notification.
-  Facilitate tracking of follow-up for referrals and missed appointments, and conferencing between clinical staff and community-based care coordinators.
-  Initiate, coordinate and conduct community outreach activities.
-  Build linkages & Provide Technical assistance to NGO’s/CBO’s/FBO’s for timely, coordinated access to medically appropriate levels of health and to support services;
-  Forecast and request for Care and Support commodities
-  Co-ordinate, track progress, review results and address care and support challenges.
